For the simple fallback rendering, this should be driven by the ABC width
of the double diacritic. The drawback of that method is that it must make
assumptions about the width of the characters to which the combining mark
is applied.
By making the A width = -B/2 and the C width = - B/2, the combining
character has no advance widht and its central point resides between the
two characters.
For more complex rendering, both the central point and the width would need
to be adjusted, as well as the height for use with capitals vs lower case.
