http://std.dkuug.dk/jtc1/sc2/wg2/docs/n3381.pdf states that on p1349 Annex A.1, 'insert after #139 Rejang, #140 Cyrillic Extended-A & #141 Cyrillic Extended-B'. But on p1351 Annex A.1, 'add #138 & #139 to Cyrillic'.

p.s. Does Unicode use 'Collection numbers', or is this peculiar to ISO/IEC 10646?
